Immunization

The university immunization requirements comply with Senate Bill 787, effective 2004-2005 academic year. In addition to the Senate Bill requirements the University adopted a policy regarding TB screening. SB787 requires all first time enrollees including transfer students to follow SB787 recommendations.

All international students will be screened for TB within Two weeks of arrival on campus. This applies only to their first year of enrollment. This test cannot be waived.

Immunization	Options	Other Information
Measles, Mumps, Rubella (MMR)	All entering college students born after 1957 should have 2 doses. Proof of vaccination: Lab test demonstrating immunity, Certificate of exemption signed by a medical provider, waiver	Compliance date: Must be completed by 4th week of classes
Hepatitis B	All students entering health professions must have 3 doses. Proof of completion of series or waiver.	Compliance date: Minimum of 2 shots by 6th week of class. Completion of series by 4th week of second semester.
TB test	International students entering NSU on a student visa, must obtain a TB skin test with in 2 weeks upon arrival to the campus. Call to make an appointment for this at the SHC. This is a mandatory University policy.	All records must be in English with a medical provider’s signature. Make an appointment with SHC upon arrival for immunization status review.
Influenza	All students regardless of age or medical status should receive the Flu vaccine. CDC is now recommending the vaccine for children	SHC holds a Flu vaccine clinic, usually, between Fall Break and Thanksgiving Break. Watch for e-mails and flyers for specific dates.
Meningitis	Strongly recommended for all students living on campus. Immunity is conferred for 5 years. *Some states will not allow students to enter college without receiving the vaccine or signing a waiver confirming the have knowledge regarding this preventable deadly infection. Proof of vaccination or waiver.	SHC holds a Meningitis vaccine clinic, usually, in early September. Watch for e-mails and flyers for specific dates.
Acceptable Documentation	Copies of shot records Copies of medical records containing immunizations Copies of school health records Copies of laboratory test results demonstrating immunity	Must be legible and signed by a nurse or medical provider.
Transfer Students	Alabama, Arkansas, Idaho, Iowa, Kansas, Maine, Nevada, Montana, S. Dakota, Vermont, and W. Virginia	Students from these states will need to show proof of immunization, regardless of academic status.
